You can find the subject access request form below. A subject access request must be handed in to the school office, with two forms of identification. This is to ensure the correct person is obtaining the data and that they have a legal entitlement to do so.
Please be aware that not all subject access requests can be granted and there are a number of circumstances when data cannot be shared. You will be informed in writing if your subject access request falls into this category.
